Vaccination against Diphtheria, Tetanus, Polio and Meningitis ACWY in Year 9

The Immunisation Team will be visiting all secondary schools in Devon to vaccinate children against Diphtheria, Tetanus, Polio and Meningitis ACWY as part of the National Childhood Immunisation Programme.

Diphtheria, Tetanus and Polio are given as one combined vaccination and Meningitis ACWY is given as a separate vaccination at the same time. These vaccinations complete the routine immunisation programme (which is usually started as a baby) and provides longer-term protection against these serious diseases.

We are now inviting parents / legal guardians of children born between 1st September 2005 and 31st August 2006 to register their consent or decline for the above vaccinations.  Please complete the form even if you do not wish your child to have these vaccinations, as this will prevent us from contacting you unnecessarily.

This registration process is quick and easy. Parents can register (or decline) their child for vaccination via our secure online portal using the link below. Comprehensive information relating to the vaccinations will also be accessible via this portal.


We will be visiting your child’s school during the coming Spring/Summer Term and require your completed online electronic response by 23rd February.

Consents received after this time will still be processed, but your child may have to wait until the next scheduled visit to be seen, depending on the school visit date.

If your child has a medical condition we may need to contact you or your child’s doctor for further information before we can proceed with the vaccination.
